Are you an experienced social worker with a passion for helping patients transition smoothly from hospital to home? We are currently seeking Agency Experienced Hospital Social Workers to join the Transfer of Care Hub at our client Hospital on a temporary contract basis. This is a fast-paced, dynamic environment committed to ensuring safe, timely discharges.

• Working Hours:
  
  Full-time, 37 hours per week

• Conduct comprehensive assessments to determine patients' social care needs
• Collaborate with multidisciplinary teams to facilitate safe and efficient patient discharges
• Support patients and families through the discharge process, ensuring all needs are met
• Reduce delays in transfers of care by proactively managing case workloads
• Maintain accurate documentation and adhere to safeguarding procedures
• Work within relevant social care and hospital protocols, including knowledge of the Mosaic social care system (desirable)

• Recent experience in hospital or Transfer of Care settings (essential)
• Strong assessment, communication, and decision-making skills
• Ability to work effectively in a fast-paced environment
• Familiarity with social care systems is desirable, particularly Mosaic
• At least 3 years of relevant social work experience supported by referees
• Ability to commit to the full 2-week contract
