Service Care Solutions Ltd is seeking an Agency Experienced Hospital Social Worker to join the Transfer of Care Hub at our client Hospital on a temporary contract. This fast-paced role focuses on safe, timely discharges with hybrid working (three days in the office).
You will conduct assessments, collaborate with multidisciplinary teams, and support patients and families through discharge while maintaining documentation and safeguarding procedures.
