Lutheran Kosciusko Hospital is seeking an experienced Nurse Practitioner to join our primary care team in Plymouth, Indiana!

Lutheran Kosciusko Hospital is a 72‑bed acute care hospital located 40 miles from Fort Wayne, IN, offering a wide range of services to this family‑oriented community.

• Outpatient only – general primary care practice
• Practice call required
• Monday – Friday schedule, daytime hours
• Manage a variety of acute and chronic conditions
• Develop ongoing relationships with patients in a small community
• See patients through life span: newborn to end of life
• Collaborative relationship with primary care physician
