Social Work Resource Coordinator
The Social Work Resource Coordinator is responsible for the provision of non-clinical social work services, including resource referrals, assistance with behavioral health transfers, social determinants of health intervention, and administrative support. The Social Work Resource Coordinator functions as a member of the interdisciplinary care team providing social work expertise to assist with psychosocial barriers to care, as well as supporting throughput and readmission initiatives.
Responsibilities- The Social Work Resource Coordinator is responsible for SDOH follow ups, providing resources and arranging follow up services in the community.
- Identifying and connecting patients and families with appropriate community resources, such as financial assistance programs, housing support, transportation services, and food security programs.
- Maintaining all required documentation, following all departmental procedures.
- Providing social work assessment, identifying, and addressing urgent needs.
- Coordinating appropriate mandating reporting when a safety issue is identified. Utilizing clinical supervision as needed for complex cases.
- Collaborating with local community agencies to maintain up-to-date information regarding available resources to service the patient population.
- Serving as a liaison between patients and external agencies to facilitate access to services.
- Partnering with MSW colleagues to implement discharge plans, resource-related tasks, and administrative needs.
- Assisting to identify an appropriate disposition plan for patients demonstrating knowledge of resources including availability, eligibility, cost factors, and types of service.
- Observing relevant hospital and department policies and procedures including risk management, infection control, and safety.
- Participating in supervision with Clinical Supervisor, either in individual or group setting, as scheduled by Supervisor.
- Performing all other job related duties as assigned.
A Bachelor痴 Degree in Social Work or related field. Experience in a hospital setting or experience working as a community health worker strongly preferred.
