Job Summary
Hire Care Services is partnering with a leading skilled nursing facility in Warwick, RI to recruit a compassionate and dependable Licensed Practical Nurse (LPN).
As an LPN, you will provide high-quality nursing care to residents while working alongside a dedicated nursing team in a supportive, resident-centered environment. This is an excellent opportunity for an LPN who takes pride in providing compassionate care and making a meaningful difference in the lives of residents.
Responsibilities- Provide direct nursing care to residents in accordance with facility policies, physician orders, care plans, and safe nursing practices
- Assist RNs and other nursing staff with daily duties and resident care
- Administer medications and treatments as ordered
- Monitor residents for changes in condition, side effects, or adverse reactions and report concerns appropriately
- Perform routine nursing tasks, including vital sign checks and wound care
- Document resident care and progress accurately and in accordance with facility policies and regulations
- Collaborate with the interdisciplinary care team to develop and implement individualized care plans
- Participate in care conferences and care plan updates
- Promote residents' comfort, dignity, privacy, and independence
- Communicate effectively with residents, families, and members of the healthcare team
- Follow all applicable healthcare regulations, safety procedures, and facility policies
- Promote teamwork, professionalism, and effective communication
- Graduate of an approved Licensed Practical Nursing (LPN) program
- Current, unencumbered Rhode Island LPN license
- Current CPR certification
- Previous long-term care or skilled nursing experience preferred, but not required
- Strong clinical judgment, communication, and teamwork skills
- Ability to handle multiple priorities with professionalism and tact
