Primary Function:
The Pharmacy Team Scheduler and Care Coordinator support clinical pharmacists by managing appointment scheduling, patient outreach, and care coordination across the Care New England Health System. This role serves as a central liaison between patients, providers, and health plans to ensure timely access to pharmacy services, accurate referral processing, and seamless communication. The coordinator will optimize scheduling workflows, manage re-bookings, and facilitate authorizations and referrals for medication therapy management, chronic disease support, and Centers for Medicare and Medicaid Services / Healthcare Effectiveness Data and Information Set (CMS/HEDIS) related initiatives.
JobQualifications and Specifications
High School or GED Required;
Associate degree Preferred;
Minimum 3 to 5 Years of experience in a medical or pharmacy setting with scheduling and customer service responsibilities. Medical Terminology certificate (or knowledge of)
