Job Summary:
Accepts referrals for patients who have orders from their physician and require care and services of VNA of Care New England.
Duties and Responsibilities:- Troubleshoots Intake referral issues.
- Functions as intake liaison to other VNA departments and referral sources.
- Accepts referrals directly from hospitals and community referral sources.
- Accepts all referral-related patient medical and insurance information.
- Registers patients and monitors referral source activity.
- Oversees insurance verification and prior initial authorization.
- Serves as clinical resource for clerical intake staff.
- Acts as a gatekeeper by screening patients for eligibility risk management and safety at home.
- Enters original orders.
- Performs all other related duties as assigned.
- Demonstrates specialized level of knowledge and thorough understanding of the practices/procedures related to home health care equipment/supplies home care nursing practices medical insurance requirements and reimbursement practices.
- Effectively plans work considering time supplies and support required from other individuals and departments.
- Completes job responsibilities within required timeframe according to established schedules or workflow requirements.
- Displays an ability to evaluate areas in need of improvement and provides input in order to improve current methods services programs or technology.
- Reviews and assesses information before making decisions and solving problems.
- Uses proper judgment and knowledge of established practices and procedures when addressing problems or issues.
- Maintains quality levels and standards as they relate to providing direct and indirect patient care.
- Provides appropriate information or instructions to patients’ family/visitors.
- Graduate of an accredited nursing program, diploma and/or BSN required. Bachelor’s Degree in Nursing required.
- Three (3) years of job-related experience or equivalent required. Community Health experience required.
