The successful candidate will be responsible for delivering high-quality practice nursing services within their scope of practice, with a strong focus on health promotion, disease prevention, and patient-centred care. The role includes the assessment and management of long-term conditions, delivery of national immunisation programmes, cervical cytology, wound care, and supporting infection prevention and control standards.

The Practice Nurse will work collaboratively within the multidisciplinary team to provide safe, effective, and compassionate care. Regular attendance and active participation in weekly clinical meetings is essential, contributing to service development, continuous quality improvement, and the delivery of excellent patient outcomes.

Warwick Gates Family Health Centre is a friendly, forward-thinking, and supportive GP practice serving a growing population of approximately 12,500 patients. Our experienced clinical team includes 3 GP Partners, 5 Salaried GPs, 3 Advanced Nurse Practitioners, 2 Practice Nurses, 1 Nursing Associate, and 1 Healthcare Assistant, supported by a dedicated multidisciplinary team. We work closely together, encouraging shared learning, professional development, and continuous quality improvement to ensure the best outcomes for our patients.

Job Purpose

The Practice Nurse will provide high-quality, evidence-based nursing care to the practice population, working both independently and as part of the multidisciplinary team. The post holder will deliver a wide range of clinical services within their scope of practice, supporting health promotion, disease prevention, chronic disease management, screening programmes, immunisations, wound care, and the continuous improvement of patient services.

Key Responsibilities

• Deliver safe, effective and patient-centred nursing care in line with current evidence-based practice and national guidelines.
• Assess, plan, implement and evaluate care for patients with long-term conditions, including asthma and COPD.
• Undertake chronic disease reviews and contribute to achieving Quality and Outcomes Framework (QOF) targets.
• Provide cervical cytology services, promoting screening uptake and supporting appropriate patient follow-up.
• Deliver childhood, adult and travel immunisations under Patient Group Directions (PGDs) and national immunisation programmes.
• Provide wound assessment, wound management, leg ulcer care, Doppler assessments and compression therapy where appropriately trained.
• Promote health education, disease prevention and healthy lifestyle choices to improve patient outcomes.
• Support infection prevention and control activities, maintaining high standards of clinical practice.
• Recognise and respond appropriately to safeguarding concerns involving children and vulnerable adults in accordance with local policies.
• Deliver emergency first aid and basic life support when required.
• Maintain accurate, timely and comprehensive clinical records using the practice clinical system.
• Contribute to the effective management of clinical correspondence and patient workflows.
• Work collaboratively with GPs, Advanced Nurse Practitioners, Nursing Associates, Healthcare Assistants and the wider multidisciplinary team.
• Attend and actively contribute to weekly clinical meetings, audit, service development and quality improvement initiatives.
• Maintain professional registration and undertake continuing professional development in accordance with NMC requirements.

Professional Responsibilities

• Deliver care within the NMC Code and scope of professional practice.
• Participate in clinical audit, appraisal, mandatory training and continuing professional development.
• Support the implementation of evidence-based practice and local clinical policies.
