An exciting opportunity has arisen for an experienced, motivated and forward-thinking Band 7 Clinical Nurse Lead to join our Clinical Triage Service at SWFT
.

This key clinical leadership role provides clinical oversight, support and guidance to the triage team, ensuring patients receive safe, timely and appropriate care. The service reviews referrals, supports clinical decision-making, prioritises patient need and ensures individuals are directed to the right service at the right time.

As Clinical Nurse Lead, you will provide day-to-day clinical oversight, support staff with complex triage decisions, promote consistent and robust processes, and maintain high standards of care. You will work closely with the Operational Professional Lead and Locality Manager to support the effective running and development of the service.

The role includes contributing to service development, clinical governance, audit, supervision and staff support, ensuring the service remains responsive, safe and patient-focused.

We are looking for an experienced clinician with excellent assessment, communication and decision‑making skills who can work confidently in a fast‑paced environment. You will be approachable, organised and proactive, lead by example and maintain effective clinical oversight.

This is an excellent opportunity for a skilled and compassionate clinician to help shape and develop a high‑quality Clinical Triage Service.

The post holder is required to be a Registered Nurse with substantial acute/urgent/frailty experience of managing patients with complex needs and holding advanced clinical assessment skills.

To put the patient at the centre of all decisions, reinforcing the concept of a more streamlined patient journey, improving the quality of care.

Monitor and lead improvements to standards of care through, supervision of practice, clinical audit, evidence‑based practice, teaching and supporting professional colleagues and the provision of skilled professional and clinical leadership.

The post holder will exercise advanced clinical expertise, levels of judgement, discretion and decision making in clinical care.

To work closely with Medical and Nursing staff in both primary and secondary care, to ensure patients receive appropriate investigation, intervention and treatment with minimal avoidable delays, whilst improving the quality of care.

The ICC Clinical Nurse Lead is responsible for the clinical triage of the presenting complex health needs of an identified population and for the identification of response required The post holder will be a registered nurse and They will be providing clinical leadership for all patients triaged within the ICC service. They will focus on ways to achieve and demonstrate overall improvements in health care and timely access to services.
