Social Worker - Adult Mental Health - Charnwood, Leicestershire County Council

Our client, Leicestershire County Council, is seeking an agency social worker for its Adult Mental Health team. This is a temporary ongoing role paid @ £34 per hour. It covers the Loughborough/Charnwood area of Leicestershire, although the office base is County Hall. Details of the role:
Charnwood Mental Health and Reablement team Manage a complex caseload of adults with mental health needs, undertaking Care Act assessments, reviews, risk assessments, and support planning. Act as Care Coordinator where appropriate, working closely with NHS Mental Health Services, Community Mental Health Teams, carers, and partner agencies to deliver person-centred outcomes. Undertake safeguarding enquiries, Mental Capacity Act assessments, best-interest decision making, and complex risk management to ensure people are supported safely.

Promote recovery, independence, social inclusion, and access to community resources, while supporting effective hospital discharge and preventing unnecessary admissions. Prepare high-quality assessments, reports, and case records, and contribute to multidisciplinary meetings, reflective practice, and team duty functions. Hybrid role. Attendance at the office is required for group supervision, reflective practice, team meetings and office-based duty on a rota basis when required.

Qualified Social Worker Fully Social Work England registered Must have significant post-qualified experience in adult social care Must have experience working within Adult Mental Health services or a Community Mental Health team. Strong working knowledge and practical experience of the Care Act, Mental Capacity Act, safeguarding adults, risk management, and complex case work.
