Social Worker – Case Manager
Washington, DC
Pay From: $24 per hour
- Minimum 5 years of relevant case management experience in mental health, physical health, addiction, aging, or trauma.
- High school diploma required;
Bachelor’s degree in Human Services, Social Work, Psychology, Criminal Justice, or related discipline preferred. - At least 2 years of case management experience providing services to individuals in the DC area.
- Knowledge of DC social service resources, compliance requirements, and HMIS.
- Excellent written and verbal communication; computer proficiency with Microsoft Word, Excel, and database/ HMIS systems.
- Independent, self‑motivated, friendly, professional demeanor.
- Develop and maintain a safe and supportive environment for clients.
- Coordinate with the Lead Case Manager and partner agencies to refer or solutions for housing, financial, employment, recovery, and mental health needs.
- Bi‑weekly face‑to‑face case management meetings with clients or schedule regular contact as outlined per contract.
- Maintain regular communication with the Lead Case Manager regarding client successes and challenges.
- Collaborate with partner agencies and other professionals to coordinate client needs and preferred treatment methods.
- Participate in organization‑wide meetings, departmental meetings, team meetings, and trainings as assigned.
- Present psychosocial needs, strengths, and concerns of clients during supervision, interdisciplinary meetings, and care coordination meetings.
- Create and maintain comprehensive case notes, including legal documentation, assessments, medical reports, referrals, educational updates, and service updates.
- Provide witness testimony for clients in court appearances for status cases based on mental health concerns.
- Report all serious incidents, allegations, grievances, and sensitive situations to the Program Manager immediately.
- Complete incident reports as needed.
