Description:
The Patient Care Coordinator RN oversees the continuum of care for patients, ensuring quality and efficiency throughout hospitalization and discharge. Responsibilities include coordinating treatment plans, facilitating communication between teams, managing resource utilization, and supporting patients and families with education and follow-ups.
