Overview
We are seeking a dedicated Registered Nurse to support inpatient discharge coordination, outpatient follow-up, and neurology clinic operations. This role is responsible for monitoring recent discharges, completing timely patient outreach, arranging follow-up appointments, and ensuring continuity of care. The RN will also provide clinical support in the clinic, assist physicians as needed, and manage telephone triage/MyChart inquiries.
Key Responsibilities
- Monitor inpatient discharges and contact patients within two days for care coordination, including scheduling specialty/PCP visits, arranging social work or home care, and completing medication reconciliation.
- Ensure all summary-of-care documentation is available prior to follow-up visits.
- Provide patient education, counseling, and clinical support; escalate urgent concerns to providers.
- Perform telephone triage and respond to MyChart messages.
- Assess patient health history, psychosocial factors, and support systems to develop individualized nursing care plans.
- Evaluate and document progress toward established goals.
- Collaborate with multidisciplinary providers to implement comprehensive care plans.
- Assist physicians with procedures, treatments, patient assessments, and teaching.
- Support clinic operations, supply ordering, and teamwork with the clinic manager.
- Active DC RN License
- BLS/ACLS
- Bachelor's degree in Nursing or Associate degree/Diploma in Nursing
- 2+ years of relevant clinical experience
- Neurology experience strongly preferred
- Nurse navigation experience
- Experience working closely with multidisciplinary care teams
- Detail-oriented, pleasant, and collaborative team player
- Strong communication and patient-interaction skills
