Qualifications:

Education:

Masters Degree from an accredited physical therapy program

Experience:

Experience working directly with patients in a long term care or hospital setting Licenses/Certifications/Registrations:
Current Physical Therapist state license, Current Basic Life Support (BLS) or ability to obtain within 6 months of hire. Knowledge/Skills/Abilities

Required:

Possess documentation skills, have excellent verbal communication, provide satisfactory patient services, be a team player, Demonstrates knowledge and skills necessary to provide are appropriate to the age of the patients served.

Position Summary:

A Physical Therapist plans and executes rehabilitation programs to help patients return to a higher level of physical function after an accident or surgery. They will often also help to manage or lessen the pain experienced by someone stricken with a degenerative condition or terminal disease. Physical Therapists help patients through a combination of physical activity, reflexology, massage and muscular and skeletal manipulation.

The PT collaborates with speech therapists and physical therapists to achieve the best outcomes for the patient. 90% outpatient, 10% inpatient. PT Responsibility:
Consult with patients to learn about their physical condition and symptoms and review their medical history and referrals from providers Diagnose movement dysfunction by listening to patients and observing them as they move about Develop an individualized treatment plan Coach patients and teach them therapeutic exercises and stretches to improve their condition Use hands-on therapy such as massage, to ease patients pain and provide stimulation to promote healing Keep detailed records relating to patient conditions, care plans, therapies, and progress.

Report progress and current status to appropriate parties, which may include family members, providers and members of the patient care committee. Performs other duties and responsibilities as assigned. Professional Communication:
Maintain confidentiality in matters relating to patient/family/staff and work environment. Interact with patients/families with a variety of developmental and sociocultural backgrounds. Provide information to patients and families to reduce anxiety and convey an attitude of acceptance, sensitivity, and caring. Maintain professional relationships and convey relevant information to other members of the health care team within facility and any applicable referral agencies.

Initiate communication with peers/patient care providers about clinical priorities for care. Relay information appropriately over telephone, pagers, and other communication devices. Initiate communication with supervisor/director about security concerns relating to medical, nursing and other healthcare workers. Maintain professional relationships and convey relevant information to other members of the health care team within facility and any applicable referral agencies. Teamwork:
Accept assignments based on priority, competencies and unit needs. Work closely with other care providers to complete the task and provide clinical assistance as needed. Report on patient conditions to those who require information, including physicians and nurses, and other staff. Initiate problem solving and conflict resolution skills to foster effective work relationships with peers. Report to work on time and as scheduled.

Professional Development:
Attend in-services, continuing education and departmental staff meetings Contribute to annual reviews of peer performance as requested by the unit/area supervisor, manager, or director. Respond to problems/opportunities toimprove care/customer service
