Physical Therapist Home Health | Palouse, Washington

Help patients regain their independence and enhance their quality of life by delivering skilled physical therapy directly in the comfort of their homes. This full-time, 13-week contract position offers the chance for a dedicated Home Health Physical Therapist (PT) to provide patient-centered rehabilitation to individuals recovering from illness, injury, or surgery.

• Monday Friday, 8:00 AM 5:00 PM

As part of this field-based role, youll manage a caseload of approximately 2532 points per week (typically 57 patient visits per day), utilizing the Home Care Home Base (HCHB) electronic documentation system and OASIS for comprehensive patient assessments.

• Complete thorough physical therapy evaluations, including OASIS assessments as required.
• Create and execute individualized care plans targeting mobility, strength, safety, and independence.
• Deliver targeted interventions such as therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention strategies.
• Educate patients and caregivers on exercise regimens, safe mobility, and use of assistive devices.
• Collaborate with a multidisciplinary team including nurses, OTs, SLPs, and social workersto ensure seamless, coordinated care.
• Maintain precise, timely documentation and compliance with all regulatory standards.
• Attend interdisciplinary meetings and participate in ongoing professional development.
• Daily travel within the assigned territory; mileage reimbursement is provided.

• Active PT license (in good standing) in the state of practice.
• Minimum of one year of recent Home Health PT experience, including OASIS use.
• Familiarity with HCHB preferred.
• Reliable transportation, valid drivers license, and auto insurance.
• Strong clinical assessment, communication, and organizational skills; able to work independently.

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
