Summit Home Care and Hospice is searching for an experienced and motivated Physical Therapist (PT) to join our team. We’re committed to providing comprehensive healthcare services in the comfort of our patients’ homes to positively impact quality of life by redefining the delivery of care.

• Hours:
  
  20 Hours/Week (Flexible Scheduling)
• Coverage locations:
  Washington, Indiana & surrounding areas
• Travel:
  Required + Mileage reimbursement
• Work setting:
  In‑home patient visits, training on‑site at our local office, and remotely
• Compensation:
  Pay per visit (rates vary based on visit types)

• Help patients regain mobility and independence
• Deliver physical therapy services per physician’s care plan and state regulations
• Assess patient mobility, strength, and rehabilitation potential
• Design and implement personalized therapy programs
• Educate patients and caregivers on therapeutic techniques and equipment
• Collaborate with interdisciplinary teams to ensure holistic care
• Supervise PTAs and home health aides as needed
• Document progress and participate in discharge planning

• Active Indiana PT license
• Degree in Physical Therapy (Bachelor’s, Master’s, or Doctorate)
• CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
• Recent TB test

• 2+ years of PT experience
• Home health or community‑based care experience
• Strong communication and organizational skills
• Ability to build rapport and lead with empathy
