Hospice Nurse Practitioner-Warren County Washington, NJ

We are looking for an experienced Per Diem Nurse Practitioner to provide face-to-face assessment care to hospice patients in the Warren County, NJ region. You will apply medical techniques and principles under the supervision of a physician and care for a diverse set of patients with chronic diseases.

• Conduct visits with hospice patients in and near the Warren County, NJ region.
• Work independently under the supervision of a physician.
• Complete physical exam and review of systems and assess eligibility for hospice.
• Speak with patients, families, and caregivers during visits and as needed.
• Attend IDG meetings when possible.
• Prescribe medications, supplies, and appropriate medically necessary interventions.

• Valid Nurse Practitioner license in the state of NJ.
• Strong people and rapport building skills with a positive and personable demeanor.
• Demonstrated collaboration skills.
• Strong and confident presentation skills.
• 1–2 years of primary care experience with older adults.
