Pay: $44–$47 per point

As a Home Health LPN at Ancora, your day begins with a review of your schedule and patient care plans. You will provide one‑on‑one patient care in their homes, including taking vital signs, administering medications, wound care, and monitoring symptoms. You’ll meticulously document patient progress, educate patients and caregivers, and collaborate with RNs, physicians, and the care team to communicate any changes in patient conditions.

- Clinical nursing skills: vital signs, medication administration, wound care, infection control
- Assessment and observation skills to recognize changes in patient condition
- Effective communication with patients, families, physicians, and care team members
- Time management and organization to manage multiple tasks and patient needs
- Compassion and empathy in patient‑centered care
- Documentation accuracy and familiarity with electronic medical records (EMR)
- Ability to follow care plans and clinical protocols
- Problem‑solving skills and sound clinical judgment
- Adaptability and independence in home or community settings

- Experience in home health or hospice (preferred)
