Case Manager at Connecticut Renaissance

This role provides educational and vocational assistance to clients in Connecticut Renaissance's DOC funded programs and serves as a liaison between clients and employers.

Responsibilities
  • Facilitate a weekly LAI – Life After Incarceration group for all clients.
  • Facilitate a weekly employment readiness class for unemployed and under‑employed clients, including mock interviews and interview basics.
  • Ensure all clients have updated resumes and provide referrals to outside resources for training, education, and employment.
  • Initiate contact with prospective employers or agencies to establish employment leads.
  • Transport clients to job interviews, job fairs, and other employment‑related activities.
  • Conduct weekly employment verification phone calls per DOC contractual requirements, using the Program Director’s approved templates.
  • Complete weekly employment progress reports for unemployed/under‑employed clients.
  • Prepare a comprehensive monthly report (by the 5th of the month) detailing client employment status, group attendance, hires, pay rates, resume completions, and interview transports.
  • Perform other duties as assigned, including Counselor on Duty coverage.
Knowledge & Abilities
  • Proficiency with Electronic Health Record systems.
  • Understanding of HIPAA regulations and related laws.
  • Strong general office software skills, particularly Microsoft Office Suite.
  • Effective client and coworker interaction.
  • Analytical and problem‑solving skills.
  • Clear verbal and written communication.
  • Ability to manage priorities, meet deadlines, and multitask.
Education & Experience
  • Associate’s Degree in Human Services, Criminal Justice, Business Administration, or a related field;
    Bachelor’s degree preferred.
  • Experience conducting effective meetings and presentations with business community members.
  • Experience in job development/placement in private or public settings.
  • Experience working in a residential setting.
Other Requirements
  • Ethical, self‑motivated, dependable, and reliable.
  • Ability to interface effectively with clients, staff, senior management, referral organizations, and potential employers.
  • Physical ability to perform routine office tasks, including climbing stairs and carrying objects up to 20 pounds.
  • Valid driver’s license.
