Job Description

Per diem opportunity for an Occupational Therapist to join an established and growing home health care agency. Most work is remote and in the homes of patients. Clinicians are paid for each completed visit and documentation. The work schedule is dependent on patient needs. This is an excellent opportunity to work independently providing care to patients in their homes. Therapists work collaboratively with nurses and other disciplines providing care, education, and support to patients and their caregivers.

• Perform home visits for patients in a home care setting.
• Regularly re-evaluates the therapy needs of our clients; initiates, develops, implements, and makes necessary revisions to our patient's plan of care.
• Observes, records, and coordinates care with multi-disciplines regarding the patient's treatment and any change in the client's condition.
• Teaches, supervises, and counsels the patient and family regarding home therapy procedures and other care needs as appropriate to the patient's condition.

• Current license as an Occupational Therapist in the state of Connecticut.
• Valid Driver's License
• Reliable transportation and willing to travel in New London County and other surrounding towns.
• Strong communication and interpersonal skills.
• Home health experience including Medicare preferred.
