As a full or part-time Home Health Physical Therapist, your days will be filled with meaningful interactions and opportunities to make a real difference in the lives of our patients. You will conduct thorough patient evaluations to determine their physical condition and needs, develop and implement individualized care plans, and provide skilled physical therapy services in the comfort of patients  homes.

Education and patient advocacy will also be essential components of your role as you work towards helping patients achieve their health and mobility goals. Additionally, you will be responsible for maintaining accurate and up-to-date patient records and documentation.

• Graduation from a physical therapy program approved by The American Physical Therapy Association, the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council on Medical Education of the American Medical Association in collaboration with the American Physical Therapy Association
• Registration in Connecticut

Home health experience and experience with OASIS are helpful.
