Are you passionate about enhancing lives? We are seeking a Physical Therapist to join our team!
As a Physical Therapist, you will be responsible for directly participating in physical therapy treatment interventions and services, in accordance with state practice guidelines. This position provides direct patient care to primarily adult and geriatric patients.
Essential
Key Job Responsibilities
- Provides screenings and evaluations to determine requirements for skilled physical therapy intervention relative to patient’s prior level of function. This includes but is not limited to the following areas: gait and balance, mobility, range of motion, strength, pain, conducting home assessments, discharge planning, development of maintenance programs and other related patient care issues.
- Develops and implements treatment plans that focus on functional and measurable outcomes and adheres to professional standards of practice.
- Monitor patients' progress and adjust treatment plans as needed.
- Provides appropriate supervision for physical therapist assistants in compliance with licensure, regulatory and company requirements.
- Maintain accurate and up-to-date patient records.
- Collaborate with, coordinate, and participate in training programs for community staff, patients, families, and rehabilitation staff.
- Participate in continuous quality improvement activities.
Education and Experience :
- Successful completion of an accredited Physical Therapy program.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong problem-solving and critical thinking skills.
- Compassionate and patient-centered approach to care.
- Current valid Physical Therapist license to practice in appropriate state.
