Host Home Supervisor; Contract Monitor - Eastern Iowa Region

LSI is looking for a dedicated, energetic full‑time supervisor to serve as a Contract Monitor for our Host Home Services in the Eastern Iowa regions. The position is based in Cedar Rapids, Cedar Valley, or Dubuque and offers some work‑from‑home flexibility with required travel within the service area.

Location:

Eastern Iowa – Counties include Allamakee, Benton, Black Hawk, Bremer, Buchanan, Butler, Cedar, Chickasaw, Clayton, Clinton, Delaware, Dubuque, Fayette, Floyd, Howard, Jackson, Johnson, Jones, Linn, Louisa, Muscatine, Scott, and Winneshiek. May require travel outside these counties for service needs and meetings.

• Act as the main contact for interdisciplinary team members and individuals served in the host‑home program.
• Work directly with assigned host homes to provide support, assistance, and instruction that promotes personal growth and independence.
• Facilitate and participate in interviews and evaluations related to the contracting process for independent providers.

The role is salaried, generally follows Monday–Friday business hours with occasional flexibility for evening or weekend work, and includes participation in an on‑call rotation.

• High School Diploma/GED and five years of related experience.
• Associate’s Degree and three years of related experience.
• Bachelor’s Degree and one year of related experience.

• Bachelor’s Degree in a human services related field and one year of related experience.
• Years of related supervisory or leadership experience.
