Overview

We are looking for an experienced, compassionate and motivated Staff Nurse to join our Hospice at Home team, based at Rowans Hospice in Purbrook. This rewarding role offers the opportunity to deliver specialist palliative and end‑of‑life care to patients in the comfort of their own homes. Working both independently and as part of a multidisciplinary team, you will provide high‑quality, person‑centred nursing care while supporting patients and their families through some of life's most challenging times.

Responsibilities
  • Assess patients' needs and contribute to the development, implementation and evaluation of individual care plans delivered within patients' homes.
  • Build and maintain compassionate, professional relationships with patients, their families and carers, ensuring care is tailored to their individual needs.
  • Deliver evidence‑based nursing care, undertaking clinical procedures and treatments in line with current best practice.
  • Assess when input from other healthcare professionals or specialist services is required and make appropriate referrals, seeking guidance from senior colleagues where necessary.
  • Communicate effectively with GPs, District Nurses, Hospice Doctors, the Hospice at Home Co‑ordinator and Senior Hospice Nurses to ensure continuity of care.
  • Participate in the out‑of‑hours service, reporting patient concerns and operational issues to the appropriate on‑call professionals.
  • Verify expected deaths where appropriate, following organisational policies and procedures, and provide sensitive support to families and carers.
  • Carry out last offices and ensure the safe and dignified transfer of deceased patients in accordance with community procedures.
  • Administer, order and manage medicines in accordance with the NMC Standards for Medicines Management, reporting any medication errors or concerns appropriately.
  • Respond professionally to patient and family feedback, concerns and complaints, supporting investigations where required.
  • Maintain professional registration and practice in accordance with the NMC Code and all relevant clinical standards, policies and procedures.
Qualifications
  • Excellent basic nursing care skills
  • Good communication and interpersonal skills
  • Ability to prioritise workload and be flexible to changing demands
  • Ability to share knowledge with others to develop and influence practice
  • Ability to maintain active and contemporaneous records
  • Awareness of own coping strategies
  • RGN or RN Adult Branch registration on NMC Register parts 1 or 2
  • Experience in palliative care, 1 year post registration
  • Valid driving licence and car ownership
