Clinical Team Scheduler

Schedules and coordinates patient visits for professional and paraprofessional staff. Works collaboratively with the Senior Clinical Team Scheduler. Triages phone calls for the Clinical Management Team. Assists with the ordering and management of patient, nursing and paraprofessional supplies. Assists with data management and administrative support activities. Provides exemplary customer service to all customer contacts. Performs all other duties that Management determines necessary.

Reports to the Clinical Manager.

Qualifications
  • High School diploma or equivalent.
  • Two (2) years' experience in a health care setting.
  • Previous scheduling experience.
  • Proficiency in the use of computers and software applications.
  • Prior office experience in a home healthcare setting.
  • Must be able to work in a fast paced environment.
  • Physical, mental and sensory abilities sufficient to meet performance standards with or without assistive devices.
Performance Criteria
  • Demonstrates the ability to coordinate clinical team processes.
  • Schedules and coordinates patient visits for professional and paraprofessional staff.
  • Communicates with staff to schedule patient visits within the visit frequency ordered.
  • Communicates with patients and families to coordinate their visit schedule.
  • Communicates any patient visit issues to the Clinical Managers to maintain compliance with the physician ordered plan of care.
  • Reports to the Clinical Manager and communicates any problems or issues as they arise.
  • Demonstrates the ability and organization skills to support the Clinical Team by managing patient, nursing and paraprofessional supplies.
  • Coordinates ordering billable and non-billable medical supplies for the Clinical Team.
  • Communicates with the Clinical Team and staff regarding supply utilization and management.
  • Communicates with Agency's supply vendors regarding ordering, utilization and management.
  • Inventories non billable supplies and places orders appropriately.
  • Provides the paraprofessionals necessary supplies.
  • Triages and responds to telephone calls for the Clinical Management Team and communicates the information to the appropriate clinical team members.
  • Provides exemplary customer service to all customer contacts.
  • Reports trending patterns to the Clinical Manager.
  • Demonstrates initiative and resourcefulness, self-directed and self-motivated qualities requiring minimal support from a supervisor.
  • Actively identifies new areas for self-development and continuously seeks out learning opportunities.
  • Demonstrates creative problem solving and execution, offers new ideas or tries new approaches to the job, task or problem.

  • Communicates schedule changes to manager or appropriate person and needs for assistance.
