Shift Detail
:
Monday – Friday. Covers 1 weekend per month and 1 major + 2 minor holidays per year. Towns covered:
Shelton, Derby, Ansonia, Oxford, Seymour.

Facilitate communication between the patient and the interdisciplinary team.

Develop and maintain a therapeutic relationship by providing education and promoting self‑advocacy and independence.

Utilize state‑of‑the‑art technology to document skilled services provided.

• Licensed Practical Nurse with a valid CT LPN license.
• One year of LPN experience; home‑care experience preferred.
• Valid driver’s license and reliable transportation required.
• Computer proficiency required.
