We are seeking an experienced and motivated Advanced Clinical Practitioner (ACP) to join our Unplanned Care Services across South and West Hertfordshire. This is a 12‑month fixed-term opportunity offering the chance to make a significant contribution to the delivery of high-quality urgent and unplanned care services.
You will work as part of a highly skilled, multidisciplinary team, alongside Advanced Clinical Practitioners, a Nurse Consultant, nurses, paramedics, therapists, GPs, support workers and administrative colleagues. Our services are focused on providing timely, high quality care in the community, supporting patients to remain at home wherever possible and facilitating earlier discharge from hospital.
As a senior clinical decision maker, you will play a key role in supporting clinical teams and strengthening partnerships with internal colleagues, primary care, and nursing and residential homes.
