The purpose of the role is to provide high quality specialist palliative nursing care, skills, advice and support to patients, carers and those important to the patient.

This role will support the delivery of planned care across Rennie Grove Peace Hospice.

This role will suit someone whois an experienced palliative care nurse who would like to be part of an outpatient service which aims to ensure equity of access to Rennie Grove and Peace services.

Main duties of the job
  • Demonstrate clinical excellence and maintain consistently high standards of nursing practice in palliative care.
  • Act as a role model providing clinical leadership for colleagues.
  • Manage a caseload of complex patients, maintaining an approach that incorporates physical, psychological, social and spiritual aspects of care.
  • Promoting the highest possible standards of individualised care, symptom management and psychological support including the delivery of practical nursing care and monitoring and evaluating the care provided.
  • Ensure all written and verbal communication is accurate and legible in accordance with NMC and the hospice policies and guidelines.
  • As a Non-Medical Prescriber, adhere to prescribing guidance for palliative care patients adhering to Rennie Grove and Peace Hospice Care policies and guidelines.
  • Be responsible for developing and maintaining effective working relationships with community professionals, support services and members of other hospices and palliative care teams, providing clinical expertise, specialist advice and support as requested.
  • Demonstrate an evidence-based approach to nursing participating in approved research projects.
  • Identify risk involved in patient care, escalating concerns where appropriate and implementing learning outcomes from recorded incidents.
  • Be responsible for identifying and acting on any risk of safeguarding to patients and their families in accordance with the Safeguarding Policies and Procedures.

At Rennie Grove Peace Hospice Care, our unique care provides a lifeline of compassion and support to patients, carers, and families facing progressive life-limiting illness. We provide palliative and end of life care support, including bereavement services, within Buckinghamshire and Hertfordshire and our patient services are available to adults, as well as patients transitioning into adult services (16 to 25 years.)

Bereavement support is offered for all ages to families of patients and those around them, as well as throughout the community, in schools and workplaces.

We are proud to offer care in the place that best suits the individuals needs, whether that's at home, in a care home, in a short stay in our inpatient unit, out in the community, or in our purpose-built Living Well Centre facilities. As a hospice care charity, we have offered this support, free of charge, for over 40 years.

Our staff team of 340+ includes a wide range of patient-facing disciplines plus operational support based in Watford, Tring, St Albans, Chalfont St Giles and Berkhamsted. The Charity is now one of the larger UK hospice charities, reaching a GP population of 797,037 of whom 75% live in Hertfordshire and 25% in Buckinghamshire.

Job responsibilities
  • Actively participate in the development of the organisations planned model of care.
  • Demonstrate specialist skills and expertise in palliative care and offer advice, support and education to the primary health care team as part of an interdisciplinary team.
  • Manage specialist planned care to a palliative nursing caseload whilst delivering high standards of personalised care.
  • Engage in the full range of activities consistent with the role of a Clinical Nurse Specialist including Nurse-Led Clinic, Assessment Clinic, Palliative Care Referral Centre (PCRC) triage, community and Living Well services as required.
  • Improve patient outcomes and experience by providing physical and emotional support to patients and carers via coordinated care and informing and advising them on clinical and practical issues associated with their illness.
  • Participate in activities such as audits and case reviews in order to improve the service.
  • Act as a Rennie Grove and Peace Hospice Care ambassador within the community, actively demonstrating our values and positively promoting the specialist care we provide.
  • As required, work across Rennie Grove and Peace Hospice Care sites (Watford and St Albans), or externally as required, to support safe and effective service delivery.
Person Specification Qualifications
  • - First level Registered Nurse
  • - Degree Level or modules in Palliative Care or another relevant subject
Additional Criteria
  • -Evidence of continuous personal and academic development
  • -Nurse prescriber/commitment to successfully achieve this qualification as agreed with line manager
  • -Advanced assessment skills training/commitment to attend this qualification
