The post holder is required to carry out a range of responsibilities including the assessment, development, implementation, and evaluation of care programmes and the management of a defined caseload of people with complex mental health and social care needs.
Responsibilities- Assess care needs and set standards of care and management for the caseload.
- Develop, implement, evaluate and document specialist mental‑health care programmes.
- Supervise, develop and teach junior staff and students, including mentorship and preceptorship roles.
- Manage the patient caseload in consultation with medical, nursing and other professionals.
- Carry out specialist mental‑health care assessments and provide ongoing interventions as indicated.
- Administer, store and dispose of medication and equipment (e.g., syringes, needles) in line with Trust policies.
- Deliver person‑centred, recovery‑focused care that reflects best practice.
- Identify physical needs of people with mental health problems and refer appropriately.
- Use research findings to formulate nursing care plans and promote evidence‑based practice.
- Support carers of service users through self‑help groups, respite care and Carers Act strategies.
- Participate in adverse incident reporting systems and comply with risk‑management procedures.
- Valid NMC registration (Professional Registration, RMN or Dip/BSc(Hons) in Mental Health Nursing).
- At least 1 year post‑registration experience at Band5 in a relevant mental‑health setting, or participation in the Trust’s preceptorship scheme if newly qualified.
- Some experience working in a healthcare setting.
- Experience with acute mental‑health problems in a community setting.
- Knowledge of the Mental Health Act
1983, CPA, risk assessment and Section
25. - Effective multi‑professional team communication and teamwork skills.
- Car driver with a good driving licence; access to a car is required unless an applicant has a disabling condition that prevents driving as defined by the Equality Act
2010. - Appropriate UK professional registration.
- Capacity for the physical effort required by the role.
- Experience working with acute mental‑health problems in a community setting.
