Memory Nurse

An exciting opportunity has arisen for an enthusiastic and committed Band 6 Memory Nurse to join our Countywide EMDASS Team. We are looking for a friendly, enthusiastic and self‑motivated individual to deliver services for West Hertfordshire as part of a multi‑disciplinary team that includes team secretaries, dementia nurse specialists, deputy community manager, service manager, psychologists, psychology assistants, occupational therapists, psychiatrists, GP, and partner agencies.

Excellent organisational and communication skills alongside solid clinical experience are essential for this post.

• Undertake memory assessments for service users suspected of dementia, deliver diagnosis in collaboration with the multi‑disciplinary team and provide post‑diagnostic support through titration reviews.
• Work between the One‑Stop Pathway and Primary Care.
• Carry out risk assessments and implement risk‑management plans with service users and carers, following safeguarding policy and protocol to ensure safety in the community.
• Collaborate with partner agencies, making referrals tailored to the needs of service users and carers to support living well with illness.
• Act as Duty Nurse, screening referrals, offering advice and signposting.
• Supervise junior staff and trainees as delegated by the MATS Team Leader.
• Support the transformation of the EMDASS service to improve pathway efficiency and service delivery.
• Provide clinical leadership and mentorship to students and junior staff, fostering an effective learning environment and remaining flexible to meet service needs.

• Solid clinical experience in memory assessment and dementia care.
• Excellent organisational and communication skills.
• Ability to work independently, problem‑solve and support others.
• Strong clinical leadership and supervisory skills.
• Familiarity with safeguarding policy and risk‑management procedures.
• Experience working within a multi‑disciplinary team.
• Willingness to be flexible and adapt to the evolving needs of the service.
• Band 6 grade requirement.
