Occupational Therapist for St. Mary’s Home care & Hospice
Description: The Occupational Therapist consults, evaluates, plans and administers skilled Occupational Therapy services prescribed by a physician to homebound patients to restore function, relieve pain and prevent disability following illness, disease, injury, or surgery. Collaborates with an inter‑disciplinary team to assure patients’ needs are met, and quality of care is achieved. When acting as the patient care manager, will manage, oversee and provide primary patient care delivery to a select group of patients supporting the patient‑centered care model, assuring quality and maintaining open communication.
Interacts with any and all members of the care group both internal and external to the organization as needed.

Minimum Qualifications:
- Graduate of an approved education program – Bachelor or Master’s preferred
- Licensure as a Occupational Therapist in the state of Georgia
- One (1) year experience as a professional clinical care provider
- Current Driver’s license and reliable transportation
- CPR certification required
Desired
Experience:
- Recent home care experience preferred
- Current GA licensure in Occupational Therapy
- One year clinical experience
