Journey Care Hospice is seeking a Hospice LPN to join our growing Crisis Care team! This is a field-based position responsible for following the plan of care for our continuous care patients, providing treatments, and working collaboratively to help our patients and their families.
HOURS: Monday - Friday (3) 12‑Hour Shifts 7PM-7AM and Every Other Weekend

Wauconda, IL 60084
Salary: $26 - $38 hourly - Offer Based on Years of Experience
Qualifications- Graduate of an accredited practical nurse or a vocational nursing program
- Three years of nursing experience. Community health, hospice or medical/surgical experience is preferred
- Currently licensed as an LPN/LVN in the practicing state
- Must possess current CPR Certification
- Demonstrates good verbal and written communication, and organization skills
- Valid driver’s license with an automobile that is insured in accordance with state requirements
- Understands the philosophy of hospice concept and the needs of the terminally ill

- Provide direct patient care as defined in State Nurse Practice Act
- Implement current nursing practice following a comprehensive assessment and the plan of care
- Provide accurate and timely documentation of patient services to reflect the plan of care
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self‑plan of care
- Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
- Provide appropriate pain/symptom management. Evaluates the patient’s response to treatments/medications
