Overview
Our Physicians serve our community by providing comprehensive direct medical care and services and working with other care team members to provide quality patient-centered care to our patients. Practiced specialists in our health centers include OB/GYN, Family Medicine, and Internal Medicine. Salary determined based on qualifications and experience.

20 hours per week
- Works with staff, as a team, to provide high-quality care to our patients by identifying and assessing health concerns, examining the patient based on the health concern, and creating a plan specific to the patient needs.
- Develops and maintains an awareness of the Chronic Care Model Processes in order to reduce health disparities within our communities.
- Provides clinical consultation to other staff members and supervision of staff and students as assigned.
- Demonstrates adherence to National Patient Safety Goals and plan.
- Participates in quality improvement activities to assess, monitor and improve program services as well as mandatory in-services and drills, attends staff meetings and other training, including appropriate age-specific educational training offered on a regular basis for professional development.
- Requires graduation from an accredited school of medicine and the completion of one year of residency (internship) in an approved program.
- Requires a medical license in the State of Illinois.
- A completed residency in an approved program in a relevant specialty, with Board eligibility, is preferred.
- Post-residency experience in Primary Care is preferred.
- Must be board certified within 2 years of hire.
