Enhance Rehabilitation is currently seeking qualified Physical Therapists to join our dynamic team. As a Physical Therapist, you will be part of a growing team providing the highest quality care for patients in an SNF setting. In this role, you will evaluate and treat patients following an individualized plan of care.
Responsibilities- Providing excellent care for our patients
- Delivering excellent customer service to patients and family members
- Collaborating with facility staff to ensure excellent care for patients
- Evaluating new patients, developing plans of care, and setting measurable, functional goals
- Treating patients, answering their concerns, and educating them on injury, progress, and probable outcomes
- Supervising Physical Therapist Assistants
- Re-evaluating patient progress, revising plans of care, and resetting goals
- Completing timely and accurate documentation, including evaluations, re-evaluations, discharges, and daily progress notes
