Care Manager, Telephonic Nurse

The Care Manager, Telephonic Nurse assesses and evaluates members' needs to achieve and maintain optimal wellness by guiding members and families toward appropriate resources. This role requires independent decision-making and critical thinking skills in a telephonic environment.

• Pay: $71,100 - $97,800/year
• Location: Remote, IL
• Schedule: Full-time
• Work Style: Telephonic engagement
• Travel: Occasional travel to Humana's offices for training or meetings may be required.

• Assess and evaluate members' needs and requirements.
• Guide members and families toward appropriate resources.
• Manage a member's physical, environmental, and psycho-social health issues.
• Identify and resolve barriers that hinder effective care.
• Monitor patient care through assessment and active care planning.
• Navigate multiple systems and employ critical thinking skills.
• Make decisions regarding work methods with minimal direction.
• Perform health assessments and education on chronic conditions.
• Facilitate interaction with resources for member well-being.

• Bachelor’s degree required

• 3+ years of clinical acute care experience required

• Licensed Registered Nurse (RN) in the state of Illinois with no disciplinary action required
• Certified Case Manager (CCM) preferred
