Youth​/Teen Mentor

• provides support and program leadership for youth ages 11-17 who participate in teen reach services;
• overall performance and implementation of teen reach programming
• assists teen reach program coordinator by formulating and coordinating individual needs assessments, goal setting, face-to-face and phone conversation on a weekly/bi-weekly basis, family engagement, linkage with community agencies, and follow-up services for youth and participants as needed;
• works to assist youth to determine what factors put them at risk of engaging in or becoming victims of aggressive behavior and other anxieties;
• helps to determine what methods each youth should use in order to avoid risk- taking factors and reach their goals;
• reaches out and re-engage youth that have inconsistent attendance, work with families to bring resources to the home, and connect with programs;
• conducts data collection and coordinate reporting to funders, marketing staff, and other stakeholders;
• utilizes the illinois department of human services (idhs) required ecornerstone internal data system to ensure program data are properly entered and exported for program and administrative needs.

• implements other services requested by sga or idhs;
• enters case notes, assessments, and case plans in a timely manner as well as responds to e-mail communication, and all other required information;
• assists with data entry tasks, including transferring paper-based files to electronic databases;
• updates and organizes files as needed;
• assists with basic maintenance of agency data system and demonstrates strategies learned for best usage of ecornerstone (if time allows);
• displays sensitivity and responsiveness to cultural differences present in the service population;
• reaches out and re-engages youth that have inconsistent attendance, works with families to bring resources to the home, and connects with programs;
• reviews monthly data reports and assesses appropriate benchmarks;
• researches and develops recommendations for data visualization tools and solutions to aid program and administrative staff in using and interpreting programmatic data;
• coordinates “data dialogues” with program staff as a part of quality improvement efforts.

• bachelor’s degree from an accredited school in social work, psychology, public health or related field, and two years of experience working with at-risk youth;
• displays sensitivity and responsiveness to cultural differences in service population.
• computer literacy skills including data entry and microsoft packages.
• must have valid driver's license and auto insurance- must be listed as a covered driver under policy.
