Pro Health Care is inviting experienced advanced practice providers (NP or PA) to join our family medicine team at the Moreland Family Medicine clinic. This position offers 0.7 FTE (approximately 24.5 weekly hours) on 1st‑shift days.

As an Advanced Practice Provider at Pro Health Care, you will take an active role in identifying and creating opportunities to educate, motivate, and collaborate with staff and students in their professional development. You will render advanced clinical practice services to patients of all ages under a collaborative agreement with a designated PHMG physician, including assessment, diagnosis, treatment, and referral when necessary.
Responsibilities- Assessment, diagnosis, and treatment of patients under your care, managing both acute and chronic illnesses with comorbidities in the ambulatory setting.
- Obtain medical histories and ensure documentation in accordance with PHMG standards, Wisconsin state law, PHMG Scope of Practice, and collaborative agreements.
- Interpret and evaluate patient data to determine management and treatment plans based on evidence‑based practice, current clinical guidelines, and medical processes; order and interpret diagnostic studies as trained.
- Perform minor surgeries and routine procedures.
- Encourage patient involvement through counseling, instruction, and education, promoting active discussion about care plans, treatment options, outcomes, preventive measures, and self‑care skills.
- Coordinate referral services, managing ancillary services, community resources, and pre‑operative plans.
- Prescribe, recommend, and regulate pharmacologic and non‑pharmacologic therapies, including controlled substances, as warranted.
- Adjust patient care plans by monitoring treatment outcomes.
- Master’s degree in Nursing (minimum education level).
- Licensed Nurse Practitioner.
- Certification by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP) in Family Practice or Adult Primary Care.
- Certifications:
NCCPA, DEA, CPR (BLS and/or ACLS).
