The Senior Health Center is a memory diagnostic clinic that provides short-term wrap around care to patients (65 and older) and their families after diagnosis. The Senior Health Nurse Practitioner works collaboratively with the medical director and other providers and renders advanced clinical practice services to adults 65 and older.

Responsibilities include:

assessment/vital signs, diagnoses, medication review/management and treatment, and providing education and support for those coping with varying degrees of cognitive impairment and associated behaviors.

• Minimum Education Level - Master's degree in Nursing
• Licensed Nurse Practitioner Required
• Certification by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP) in Family Practice or Adult Primary Care
• Certifications:
  
  NCCPA, DEA, CPR (BLS and/or ACLS)
