Community Care is hiring a RN Case Manager to join our Family Care Program team. This is a full‑time opportunity working out of our Waukesha location. $8,000 Sign On Bonus

Completes discipline‑specific initial and ongoing assessments. Conducts home visits and home assessments as needed, and consults w/ families and other supports for additional information as needed.

Develops and maintains care plans, including members' long‑term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports. Follow up on all services to ensure quality/coordination and effectiveness of services.

Education:

Associates degree in Nursing Required, Bachelors Degree preferred.

Licensure:
Wisconsin Licensed Registered Nurse

Licensure:
Eligibility for long‑term care functional screener certification is required

Licensure: WI Driver’s License

Experience:

Required : 2 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity.
Preferred : 2 years of work in community nursing

Monday‑Friday 8:00 AM‑4:30 PM.
