St. Croix Hospice is seeking a dedicated Hospice Transition Nurse to support hospital patients, families, and staff. You will coordinate hospice admissions and act as a key liaison to sustain and grow referrals across hospital and community settings.
The role requires an active RN license and hospice/home care experience, with ability to travel as needed. You will educate referral sources, lead goals of care discussions, and mentor junior nurses while coordinating care with hospital teams.
