The Professional Services Group, Inc. (PSG) is seeking a Service Coordinator for the Children’s Long-Term Support (CLTS) Waiver Program.
You will provide case management and service coordination for families and individuals needing counseling and referrals for disabilities. Responsibilities include receiving referrals, conducting assessments, developing casework plans, maintaining caseloads, coordinating various services, and performing in-person home visits.
