• Perform daily documentation of resident care, status, and changes in condition accurately and completely in compliance with facility policy and regulations.
• Transcribe physicians’ orders to Medication Administration Records (MARs) and treatment sheets, ensuring meticulous record‑keeping.
• Notify the facility pharmacy promptly of new or changed orders.
• Prepare and administer medications and perform treatments as ordered, strictly within the LPN scope of practice in Wisconsin.
• Monitor residents’ physical and emotional well‑being throughout your shift, reporting significant changes to the Registered Nurse or Director of Nursing immediately.
• Supervise Certified Nursing Assistants (CNAs) on your unit to ensure delivery of high‑quality personal care.
• Participate actively in the interdisciplinary care team to develop and implement individualized care plans.
• Perform all other duties as assigned by the Unit Manager or Director of Nursing.

• Active and unrestricted Licensed Practical Nurse (LPN) license in Wisconsin.
• Strong organizational, communication, and basic computer skills for accurate charting.
