Key Responsibilities:

• Perform daily documentation of resident care, status, and changes in condition accurately and completely in compliance with facility policy and regulations.
• Transcribe physicians’ orders to MARs (Medication Administration Records) and treatment sheets, ensuring meticulous record-keeping.
• Notify the facility pharmacy promptly of new or changed orders to ensure continuity of care.
• Prepare and administer medications and perform treatments as ordered by the physician, strictly within the LPN scope of practice in Wisconsin.
• Monitor and assess residents' physical and emotional well-being throughout your shift, reporting any significant changes to the Registered Nurse or Director of Nursing immediately.
• Supervise Certified Nursing Assistants (CNAs) on your unit to ensure the delivery of high-quality personal care.
• Participate actively in the interdisciplinary care team to help develop and implement individualized care plans.
• All other duties as assigned by the Unit Manager or Director of Nursing.

Qualifications:

• Must possess an Active and unrestricted Licensed Practical Nurse (LPN) License within the state of Wisconsin.
• Strong organizational, communication, and basic computer skills are essential for accurate charting.
• Previous experience in a long‑term care or skilled nursing setting is a plus, but highly motivated new graduates are encouraged to apply!
