Key Responsibilities
  • Medication Management & Specialized Treatments:
    • Prepare, verify, and administer all prescribed oral, injectable, topical, and enteral medications accurately and on schedule in strict compliance with physician orders and safety protocols.
    • Perform specialized clinical treatments, including wound care, dressing changes, tube feedings, catheter care, and glucose monitoring according to established protocols.
    • Maintain precise narcotic counts and controlled substance documentation at the start and end of every shift in full accordance with federal and state regulations.
  • Proactive Health Monitoring & Change-of-Condition Reporting:
    • Conduct regular resident physical assessments, monitor vital signs, assess pain levels, and closely observe overall physical, cognitive, and behavioral health status.
    • Identify subtle or acute health status changes promptly, intervening swiftly and collaborating directly with Charge RNs, attending physicians, and family members.
  • Meticulous EHR Documentation & Order Processing:
    • Maintain detailed, accurate, and real-time clinical documentation in the Electronic Health Record (EHR) system, charting nursing interventions, care plan updates, and resident responses.
    • Transcribe physician orders to Medication Administration Records (MARs) and Treatment Administration Records (TARs), coordinating with pharmacy providers for timely order fulfillment.
  • Unit Leadership & Interdisciplinary

    Collaboration:
    • Supervise, mentor, and guide Certified Nursing Assistants (CNAs) on shift, delegating direct-care tasks, ensuring meal and hygiene schedules are met, and maintaining high standards of dignity.
    • Participate actively in shift handoff reports, interdisciplinary care plan meetings, and quality improvement initiatives alongside physical therapists, social workers, and clinical directors.
  • Regulatory Compliance & Safety Protocols:
    • Adhere strictly to state nurse practice guidelines, HIPAA privacy standards, OSHA regulations, and facility infection control policies to ensure a safe and compliant care environment.
Qualifications
  • Licensure:
    Must hold a current, active, and unrestricted Licensed Practical Nurse LPN license in the State of Wisconsin (or multi-state NLC equivalent).
  • Certifications:

    Current Basic Life Support (BLS) / CPR Certification required upon hire.
  • Skills:

    Exceptional clinical judgment, strong organizational abilities, precise medication administration skills, and proficiency with digital EHR charting platforms.
  • Experience:

    Previous long-term care, sub-acute rehabilitation, or skilled nursing experience is preferred; however, every high-potential Licensed Practical Nurse LPN new graduate is encouraged to apply—we offer a robust orientation and mentorship program!
