Hospice RN Case Manager: Compassionate End-of- Care

Heartland Hospice, part of Gentiva, is seeking a dedicated RN Case Manager to provide compassionate hospice nursing in patients' homes or equivalent settings. You will assess, plan, and deliver skilled care guided by each patient’s Plan of Care, coordinating with physicians, CNAs, and social workers to ensure comfort and dignity.

You will document care, educate families, and support transitions across levels of care, while maintaining regulatory compliance and high-quality standards.
