Social Worker-Hospice

As the Social Worker, you will provide medically related social services assessing individual needs/goals to maintain or enhance the psychosocial, mental, and emotional needs of residents. Knowledge of end-of-life cares and needs. You will act as a liaison between the resident, family, and interdisciplinary team. Document all resident care plan information while following State and Federal codes. Additionally, you will perform an assessment of residents at admission, upon change in condition, and/or annually to identify and seek ways to support residents  individual needs.

• Assess individual needs/goals to support residents  psychosocial, mental, and emotional well-being.
• Serve as a liaison between residents, families, and the interdisciplinary team.
• Document all resident care plan information following State and Federal codes.
• Perform assessments of residents at admission, upon change in condition, and annually to identify and support individual needs.

• Master s in social work or other related behavioral science.
• Social Work License for the state of Wisconsin.
• Three to five years  experience preferred in Hospice or Skilled Nursing.
• Physical mobility to spend 50% of the workday moving throughout the facility to visit residents under caseload.
• Ability to document resident care plans concisely and clearly.
• Ability to be patient, sensitive, and receptive to both residents and their families  needs.
• Must be available for on-call for weekend concerns or resident admission on a rotation basis.
• Attend in-services/classes concerning the social work field and maintain social work certification.
