Total Extended Care Services, a growing Physical Medicine and Rehab group, is looking for a dedicated Nurse Practitioner/Physician Assistant to join our team. We partner with our PM&R physician in the sub‑acute rehab setting to improve patient outcomes in an interdisciplinary, specialized team environment.
Benefits- Expert team: collaborate with Board‑Certified PM&R physicians and specialized NPs/PA.
- Flexible schedule and no on‑call; evenings and weekends free.
- Part‑time position with opportunities for growth.
- On‑site pain management and focus on reducing readmission rates.
- Coordination of care with medical providers, therapists, social workers, nurses, and families.
In‑person at skilled nursing facilities, part‑time position.
Responsibilities- Partner with the facility's therapy department to provide optimal rehab care.
- Communicate and collaborate with patient’s primary care team to ensure best outcomes.
- Perform timely charting/documentation (can be done from home).
- Licensed Nurse Practitioner (TN).
- Certified Nurse Practitioner.
- Licensed Physician Assistant (TN).
- Certified Physician Assistant.
- Must be licensed in Tennessee.
- Reliable transportation.
