Education

• Graduate of an accredited school of medicine (required)
• Completion of an accredited residency program (required)
• Completion of an accredited fellowship program specific to the medical specialty (required for specialty physicians)

• For specialty physicians, completed accredited fellowship in their specialty

Licensure/Certification /Registration

• Current MD license in the appropriate state
• Board certification or eligibility in Sleep Medicine
• Current DEA license (required)
