Position Overview
Upperline Health is seeking a dedicated and compassionate Primary Care Physician to oversee the overall health of patients through comprehensive, coordinated, and value-based care. The ideal candidate will play a pivotal role in improving outcomes for high-risk populations and collaborating with interdisciplinary teams to lower total healthcare costs.
This physician will serve as a trusted clinical leader, responsible for providing primary and preventive care, managing chronic conditions, and ensuring timely specialist referrals when needed. The role offers opportunities for growth, including expansion into program leadership or broader market roles as Upperline continues to scale.
Key Responsibilities
- Conduct routine exams, screenings, and immunizations
- Diagnose and treat acute and chronic conditions
- Provide education and counseling on disease prevention and wellness
- Develop and manage treatment plans, including medication and lifestyle interventions
- Collaborate with interdisciplinary care teams to ensure holistic, coordinated care
- Establish long-term relationships with patients to tailor care to individual needs
Qualifications
- M.D. or D.O. degree required
- Board certification in Internal Medicine, Family Medicine, Preventive Medicine, or Geriatrics
- Active medical license in an Upperline Health operating state
- Experience in value-based care or managed care models
- Strong communication and collaboration skills
- Commitment to patient-centered care and health equity
- Excellent organizational and multi-tasking abilities
