With over 130 podiatric medical and surgical clinics in 7 states, Upperline Health providers coordinate patients care among a team of specialists podiatric physicians and surgeons, Primary Care Physicians, Advanced Practice Providers, Care Navigators, Pharmacists, Dieticians, and Social Workers for integrated treatment that addresses patients immediate and long-term health needs. We put patients at the center of value-based care.

About the Primary Care Role
Upperline Health is seeking a well-trained and compassionate Primary Care Physician (MD/DO) to join our growing specialty care team. This position provides a unique opportunity to practice within a multidisciplinary care model designed to proactively manage primary care needs along with Chronic Disease Management. This position is a clinic-based setting with the ability to provide both consultative and longitudinal care, supported by care coordination teams and advanced technologies.

Compensation & Benefits

• Flexible schedules:
  Full-time hours are Monday Friday, 8 a.m. 5 p.m.
